Buy a case you like,
go to home depot,
find a handle you like,
pick up a small piece of wood/metal that you can use to reinforce the top of the case,
pick up a couple of nuts and bolts,
drill holes in the case and reinforcement material,
stick on the handle.
Voila.
As for cases...I'd go with something like the Lian-Li PC7 plus. You aren't going to get a quality case with a quality PSU, with the exception of some Antec models, so buck up and spend $60 on the PSU to have some peace of mind.
P.S. You ain't gonna find a case that fits all of your needs without some modifications, especially if you're looking to spend <$150. Actually, come to think of it, there is no case on the market with your specs.
Yeah, I'm not sure where you're going to find a case with a handle (without it looking fugly). Personally I like the Kingwin KT-424. It's got the Lian Li type look but seems a little cleaner than the PC-7. (no holes, just a fan mesh thingy at the front)
Talk about a **** case market. I'm not asking for anythng astronomical here other than a decent power supply.
I hate Antec cases. They weigh like 100 lbs and trap dust like a vacuum. The install slots thing is a stupid, pointless gimmick. The door breaks off as well but that's good because doors are stupid(I had a very similar antec case).
I just want a case with a lot of fan ports really.
Actually I don't need a handle. I had that universal strap handle thing. But one of the straps will probably cover and airhole.
I don't mod cases myself anymore. Last time I drilled a hole a large fracture was created in the metal somehow (sorta like a crack on a glass window but on metal). The top of a case can't always support the weight and the metal gets all bent out of shape.
