Notepad or some other text editor like ultraedit (which has that nifty "preview" button). also, i've used webedit pro which is essentially notepad + a bunch of help and wizards for tables and frames and such if you get lazy.

Notepad at the moment, till I finally get round to rebuilding my system.
I normally use Interdev which I got as part of Visual Studio 6.
