I don't remember my initial settings as I had a problem and immediately went to 1.1 which was already out. Then, shortly after, 1.3 came out and away I went.
I never ever had a problem with my floppy not working. So yours is just totally dead?
If it is, I would make a Ms-Dos Boot Disk on another comp. Then copy this disk to a CD along with the Bios files. Then just boot off the CD and "runme.bat"
I would try that and see if it brings your floppy back.

IF your floppy works buy you just can't boot off of it, just copy the bios and awdflash.exe to the floppy.
Then hit Alt-F2 at boot. See if that works. (Although I prefer manually running "runme.bat" myself)

Got desperate and used FlashMenu Online to Flash BIOS to 1.6(I've a UPS so wasn't too scared). I'm in Enhanced mode, and IDE Bus Master enabled. IDE1 = 40gb HD Master, IDE2 = CDRW Master, SATA1 = Raptor. STILL can't boot from Floppy! I've tried two Floppy Drives and two cables -- both work in my Server! If this wasn't such a biotch it'd be funny OK, so tell me all about your settings now, big boy!

Did you try the basics? *lol I used that before!!*
Did you test that boot floppy in another machine? It is possible it doesnt work anymore.. I have had many of them just go bad on me...
Did you download a bootimage from that site I listed earlier and create it on a newly formatted floppy?
